It depends entirely on the offer. It is almost never free anymore. Typically it's 3% of the balance transferred.

Some fees are still "capped" at something like $99 or $199. In that case, you're generally better off doing one large balance transfer rather than two smaller ones, since you're only subject to the capped fee one time.
