Typically accidental death and dismemberment insurance policies pay 50% for loss of a single limb and 100% for loss of two limbs.

The actual amount of the payout depends on the face value of the policy. This can vary from company to company and not just from insurer to insurer. The maximum amount from Provident is $500,000, but your company may have had lower liability limits.

You should talk to the HR office at your employer to find out about company-specific limits.

You may wish to talk to an attorney before you sign any paperwork if the amount seems low. The existence of the insurance policy does not necessarily absolve the company of liability if the company was negligent in not providing adequate safeguards or if your injury was caused by another employee.
